Male Pattern Baldness: Stages, Causes and Treatment Options

Male pattern baldness is inherited hair loss in men that follows a fixed route: the temples recede, the crown thins, and the two areas slowly join. Doctors call it androgenetic alopecia and grade it on the Norwood scale, from stage 1 to stage 7. The stage decides the treatment. Early stages are managed with medicine and PRP to keep the hair that is still there. Middle stages often add a transplant to the front. Advanced stages need careful planning around a limited supply of donor hair.

Male pattern baldness stages and the treatment that fits each

The Norwood scale describes where hair has gone, not how fast it is going. A review of hair loss classifications sets out the stages below. Surgery usually enters the plan from stage 3. An FUE hair transplant in Lahore is often the first surgical step, as it moves hair to the front without a linear donor scar.

Norwood stageWhat the scalp showsTreatment usually considered
Stage 1Little or no recession of the hairlineNo treatment, dated photos to track change
Stage 2Small, matching triangles of recession at the templesWatch closely, medicine if photos show movement
Stage 3Deeper temple recession, the first stage counted as baldness; in the vertex type, crown loss with limited recessionMedicine and PRP, then FUE to the front once loss has slowed
Stage 4Deeper front recession and a sparse or bare crown, still split by a band of hairFUE to the front, medicine to hold the crown
Stage 5The band between front and crown becomes narrow and thinA large FUE or FUT session, crown often treated later
Stage 6The bridge of hair is gone and front and crown joinFUT, FUE or both, spread over planned sessions
Stage 7Only a horseshoe of hair remains at the sides and backFront-weighted coverage within donor limits, scalp micropigmentation for the look of density

Age, speed of loss and donor density can give two men at the same stage different plans.

What causes male pattern baldness?

The cause is an inherited sensitivity of scalp follicles to male hormones. Norwood’s original work named testosterone as the trigger. In sensitive follicles, the growing phase of each hair cycle gets shorter and the resting phase gets longer. Each new hair comes out thinner and shorter until the follicle produces only a fine fuzz. The hair at the back and sides is usually spared, which is why it can be moved.

Is male pattern baldness genetic?

Yes, but not through one gene or one parent. Androgenetic alopecia is polygenic, meaning many genes add up. A 2026 meta-analysis found that a family history gave about 2.7 times the odds of having it. A father with pattern loss raised the odds about 2.2 times. Our guides to the early signs of balding and to a receding hairline help with spotting the first stages.

Treating early stages without surgery

Male pattern baldness treatment in stages 1 to 3 aims to keep the hair a man still has. The American Academy of Dermatology notes that men who start soon after noticing loss tend to do best.

  • Minoxidil is applied to the scalp. If it works, results can take 6 to 12 months to show.
  • Finasteride is a prescription tablet. When it helps, results usually start to show at about 6 months.
  • PRP (platelet-rich plasma) uses a concentrate of the patient’s own blood injected into thinning areas.
  • Laser devices for home use are FDA cleared, which is a less strict standard than the approval given to medicines.

At Mayo Hospital, Lahore, researchers gave 30 patients two PRP sessions a month apart. Mean hair density rose from about 34 to about 50 hairs per square centimetre over the following months. The study was small and had no control group, so it shows promise rather than proof. Our page on PRP hair treatment in Lahore explains how sessions are planned.

Medicine only works while it is used, and the right choice depends on age, health and family plans. The surgeon decides which medicine, if any, suits each man at consultation, and our hair growth medications service covers follow-up. If a year of treatment holds the pattern steady, a transplant can then be planned on stable ground.

How many grafts each stage needs

Graft needs follow the area covered more than the stage label. Published surgical data give these ranges:

Area to coverGrafts usually needed
Soft front edge of the hairline only600 to 900
Frontal forelock only, when donor hair is limited1,500 to 2,000
Whole frontal area3,000 to 3,500
Front and mid-front4,500 to 5,000
Full coverage of the top6,000 or more

The limit is the donor area. A study of 820 men with advanced baldness puts the permanent donor zone at roughly 10,000 to 15,000 follicular units. The authors kept extraction to 25% of that zone to leave the back looking normal, which yields about 2,500 to 3,500 grafts from the scalp. In their series, beard and body hair were added when the scalp could not supply enough.

So a stage 6 or 7 plan usually puts density at the front, where it is seen first, and accepts a lighter crown.

FUE, FUT or both for advanced stages

Both methods move the same donor hair. They differ in how it is taken out.

  • FUE (follicular unit extraction) removes grafts one at a time with a small punch. The back is usually shaved, and FUE leaves small dot scars that short hair usually covers.
  • FUT (follicular unit transplantation) removes a thin strip of scalp, which is divided into small grafts of 1 to 4 hairs. The wound is stitched, leaving one line scar that is hidden unless the hair is cut very short.

The NHS notes that a large area may need two or more sessions on different days. For a stage 6 man with a dense donor area, a plan may combine both methods across sessions to cover a large area. Our comparison of FUE and FUT hair transplant methods goes deeper, and the FUT hair transplant page explains the strip method in detail.

Who is a good candidate at each stage

A transplant suits a man whose pattern is confirmed, whose loss has slowed, and whose donor hair is dense enough for the area he wants covered. One clinical study of FUE enrolled men at Norwood stages 2 to 5 who had shown little response to medical treatment for at least 6 months.

Usually advised to wait or choose another route:

  • young men whose loss is moving fast and whose final pattern is unclear
  • men with patchy loss from alopecia areata, which a transplant does not usually suit
  • anyone with an active scalp infection or skin disease
  • men at stage 7 who expect full density across the whole top

The real question is whether donor supply can meet the demand.

Male pattern baldness myths heard in Pakistan

Myth: baldness comes from the mother’s side. The genetics are spread across many genes. The same meta-analysis linked a father’s hair loss to higher odds, so both sides count.

Myth: smoking has nothing to do with hair. Smoking was linked to having androgenetic alopecia, at about 1.5 times the odds, and to its getting worse.

Myth: there is always enough hair at the back. The donor zone is finite, as the graft figures above show. A clinic that promises full density at stage 7 from scalp hair alone is ignoring that limit.

Myth: hair loss is only a cosmetic problem. A study at Pak Emirates Military Hospital, Rawalpindi linked androgenetic alopecia with metabolic syndrome, at an odds ratio of 2.4. The authors suggested early screening for metabolic problems in people with this hair loss.

Treatment costs in Lahore and what to ask before booking

In Lahore the per-graft rate is estimated at PKR 70 to 100, so the graft count from your stage and coverage plan drives the total. These are market estimates, and the exact figure is given at an in-person, paid consultation.

Before booking, ask:

  1. What Norwood stage am I, and is that written in my notes?
  2. How many grafts does my plan need, and how many can my donor area give?
  3. Why FUE, FUT or both for my case?
  4. Who makes the incisions and who places the grafts?
  5. What will protect my crown and native hair after surgery?

Patient testimonials from men at a similar stage show how plans work out in real life.

Why choose Aneels Hair Transplant for male pattern baldness in Lahore

  • Every technique for every stage. The clinic offers FUE in manual, standard and sapphire forms, FUT with trichophytic closure, and hybrid FUE plus FUT for large areas. A stage 3 hairline and a stage 6 full top each get the method that fits.
  • Medicine before and after surgery. PRP, exosome therapy, laser hair therapy, hair growth medications, nutritional counselling and scalp micropigmentation sit alongside surgery. Early stages get medical care, and native hair is protected after a transplant.
  • Awake through the procedure. Surgery is performed under local anaesthesia. You stay awake and can watch videos or use your phone during a long session.
  • One surgeon-led clinic. Dr. Aneel Riaz Batalvi is on site from 4:00 pm to 7:00 pm. Consultants see patients from 11:00 am to 8:00 pm, and support staff are available 24/7.

Frequently Asked Questions

How do I fix male pattern baldness?

It cannot be cured, but it can be controlled and its look restored. The usual path is medicine first to slow the loss, then moving permanent hair from the back of the scalp to where it matters most. The best order depends on the stage, the speed of loss and the donor supply.

Is Gen Z going bald faster?

No reliable study has shown that this generation is balding faster than earlier ones. What research does show is that some habits common among young adults are linked to faster progression, including poor or short sleep and regular alcohol use. Genes decide who loses hair, and habits can change the pace.

Is PRP or minoxidil better for early male pattern baldness?

Neither wins on every measure. A randomised trial at PNS Shifa Hospital, Karachi compared the two in 70 patients over six months. Minoxidil did better on the hair pull test, while PRP was rated effective in more patients. Many doctors combine treatments, and the surgeon decides the mix at consultation.

What does treatment cost at each stage in Lahore?

Early stages mostly cost medicine and PRP. One PRP session in Lahore is estimated at PKR 8,000 to 20,000. Surgical stages are priced by graft count, and Lahore packages are estimated at PKR 90,000 to 250,000. Advanced stages cost more because they need more grafts and sometimes more than one session.

Do I need to stay in Lahore after a FUT session?

Plan for a return visit. Non-dissolvable stitches from a FUT strip are usually removed after 10 to 14 days. Patients from other cities come back to Lahore for that visit or ask beforehand about stitch type. FUE patients have no stitches, though the first wash and early checks still need planning.

Can a hair transplant stop male pattern baldness from getting worse?

No. Transplanted hair comes from an area that resists the condition, but the native hair around it can keep thinning. The NHS advises that a transplant should be designed to look natural as time passes. Ongoing medical treatment protects the hair that was not moved.
