Donor Area After Hair Transplant: Healing and Regrowth Guide

The donor area after hair transplant surgery usually settles within about two weeks, but it never refills. After FUE (Follicular Unit Extraction), many tiny round wounds scab over and fade. After FUT (Follicular Unit Transplantation, the strip method), one stitched line heals into a linear scar. How many follicles are taken, and how evenly, decides whether the area still looks full when you cut your hair short.

Donor area after hair transplant: what to expect week by week

This section covers the back and sides of the head only. For the transplanted area, see our full hair transplant recovery timeline. If you are still planning surgery, the donor plan is the first thing to discuss for an FUE hair transplant in Lahore.

StageDonor area after FUEDonor area after FUT
Day 1Dressing on; scalp feels tight and achyDressing over the stitched line; scalp feels tight and achy
Days 2 to 7Bandages usually off within two to five days; tiny scabs visible if the area was shavedBandages usually off within two to five days; the line is hidden under longer hair above it
Weeks 1 to 2Scabs come away, usually one to two weeks after surgeryNon-dissolvable stitches removed at 10 to 14 days
Weeks 2 to 4Shaved hair around the wounds keeps growing and starts to cover themThe line settles; follow your surgeon’s advice on washing and exercise
Later monthsSmall round scars that short hairstyles hide when the work is done wellOne linear scar, confined to one band at the back

The table shows why the two methods feel so different in the first fortnight. FUE spreads small wounds over a wide area, while FUT concentrates the healing in one line. Both methods leave a mark of some kind. Our comparison of FUE and FUT hair transplant methods explains the trade-off, so choose the method whose mark suits the hair length you like to wear.

Why the hair transplant donor area is a limited supply

The donor area is the band of hair at the back and sides that does not thin with pattern hair loss. The International Society of Hair Restoration Surgery (ISHRS) states on its FUE patient page that ruining the donor area is an irreversible problem. Its revision guidance adds that there is a limited number of hairs available, and that a lack of donor hair is one good reason not to operate.

Where grafts come from also matters. A Pakistani study of FUE in ten patients found fewer follicles were cut during removal in the middle of the back of the head than at the sides. More follicular units also sat in the central zone. A careful surgeon spreads extraction across the safe zone and respects that it is uneven. Ask your surgeon to show you, on your own scalp, where grafts will be taken from.

Donor area pain after hair transplant: what is normal

Most patients describe the donor area as sore rather than painful. The society’s aftercare page says discomfort typically lasts no longer than a day or two, followed by seven to ten days of tenderness to the touch. FUT can cause more discomfort than FUE in some cases. Your surgeon prescribes pain relief, and you should take only what is prescribed.

Some signs are not normal. The NHS hair transplant guide lists bleeding and infection among the risks. Spreading redness, pus, rising pain after the first few days, or a fever are reasons to call the clinic promptly rather than waiting for your review.

How an overharvested donor area happens

Overharvesting means taking more follicles than the donor area can spare, or taking them from too wide an area. The society names both as among the greatest risks of FUE. When punch holes are placed too close together, blood supply to the skin between them can suffer. The result can be thin, uneven patches that show as soon as the hair is cut short.

Strip surgery has its own long-term effect. A second Pakistani study looked at ten patients who had FUE after an earlier strip procedure. Their scars averaged 2.47 mm wide, and hair grew at different angles above and below the scar. During FUE, twice as many follicles were cut below the scar. So if you have had FUT before, tell the surgeon, because the donor area needs a different harvesting plan.

Caring for the donor area in Lahore heat, dust and traffic

Healing is easier when you are not sweating heavily under a dressing. Lahore’s normal mean maximum falls to 27.5°C in November, based on Pakistan Meteorological Department normals for 1981 to 2010. If your timing is flexible, a cooler month like that keeps the back of the head drier in the first week.

Dust and traffic need care too. Keep the donor area clean, wash it exactly as instructed, and avoid rubbing scabs off early. Our guide to hair washing after a transplant covers the method. The NHS notes that you may be told to cut back on exercise for the first month.

Repair options for a damaged donor area

Repair is possible, but it has to be planned with the remaining hair in mind. A poorly planned correction can add scarring and leave the donor area worse. Common routes are:

  • Revising a wide strip scar: the scar can be cut out and closed again, ideally with a trichophytic closure so hair grows through the new line. See our FUT trichophytic closure page.
  • Grafts into the scar: FUE grafts can be placed into a linear scar or into thin patches to break up the line.
  • Scalp micropigmentation: SMP (scalp micropigmentation) deposits pigment dots that mimic short hair over scars or thin areas.

Before you book any donor area repair, ask:

  1. How many grafts can my donor area still safely give?
  2. Will my scar be revised, grafted, pigmented, or a mix of these?
  3. Who will remove the grafts and make the incisions?
  4. How long should I wait after my last surgery before repair?
  5. What will the donor area look like at my usual hair length?

Repair grafts are priced like any transplant, at an estimated PKR 70 to PKR 100 per graft in Lahore, with the exact figure set at a paid consultation. Our scar revision service explains the options in more detail. Bring photos of the donor area after hair transplant surgery, at your normal hair length, so the surgeon can judge your case.

Why choose Aneels Hair Transplant for donor area care and repair

  • Donor scar repair: the clinic corrects hairlines, graft placement and donor scars left by earlier procedures, so a damaged donor area is planned around what is left.
  • Reconstructive experience: Dr. Aneel Riaz Batalvi’s listed procedures include body hair transplantation and burned scalp reconstruction, which matters when scalp donor hair is short.
  • Both harvesting methods: FUE (Manual, Standard and Sapphire), FUT with trichophytic closure, and hybrid FUE plus FUT are offered, so the method can match your donor area.
  • Camouflage under one roof: scalp micropigmentation, PRP (platelet-rich plasma) and other non-surgical treatments are available for scars and thin patches that surgery should not touch. You can see grown results on the results page.

Book a consultation to have your donor area measured and your scar or thinning assessed before any further surgery. Call 0341-7555512, message us on WhatsApp, or visit 32 B Masson Road, behind Ganga Ram Hospital, Lahore.

Frequently Asked Questions

Will the hair in my donor area grow back?

The follicles that were removed do not grow back. The hair around them does keep growing, which is why a well-harvested donor area looks normal once it is a few weeks long. The society puts it plainly: donor hair, once removed, is gone forever. That is why each extraction has to be planned for your whole life, not one session.

Will a revised FUT scar heal better than the first one?

It can, but a scar never disappears completely. Healing also depends on the patient. The society notes that scars tend to heal better with age: a 50-year-old usually gets a better scar than a 30-year-old. Your surgeon checks your skin and scalp looseness before suggesting a revision.

Can beard or body hair be used if my donor area is thin?

Often yes. FUE has widened the donor supply by allowing beard, chest and abdominal hair to be used. Not everyone has suitable body hair, so your surgeon examines it and decides where, if anywhere, it would be placed.

How can I tell if my donor area was overharvested?

Cut the back of your hair short, or ask a barber to take photos. An overharvested donor area looks see-through or patchy, sometimes described as moth-eaten, rather than evenly dense. Check the sides as well as the middle. A surgeon can measure the remaining density to confirm.

When can I wear a motorbike helmet after a hair transplant?

Your surgeon sets the date, because a helmet presses and rubs on both the donor area and the new grafts. If you ride to work in Lahore, raise it at consultation and arrange other transport for the first days. When you return to riding, choose a clean, well-fitting helmet with a soft liner.

How much does donor area repair cost in Lahore?

The cost depends on the route. Scar revision, grafting into a scar and scalp micropigmentation are priced differently, and a repair may combine them. Grafting is charged by the number of grafts placed. Ask for a written plan that prices each step, so you can compare quotes on the same basis.
